How to become a occupational therapy assistant: step-by-step route 2026

The route, start to finish

Becoming a occupational therapy assistant is a fixed sequence: an approved program, the NBCOT COTA exam, then whatever your state requires on top. Here is every stop in order, with realistic timings.

  1. 1

    Finish high school and prerequisites

    varies

OTA programs are usually at community colleges and technical schools. Expect prerequisites such as anatomy, psychology and biology.

  2. 2

    Complete an ACOTE-accredited OTA program

    about 2 years

Most students earn an associate degree, but some ACOTE-accredited programs award a bachelor's degree. You must graduate from an accredited OTA program to take the COTA exam.

  3. 3

Finish Level I and Level II fieldwork

    16+ weeks Level II

Level I fieldwork is built into coursework. Under the 2023 ACOTE Standards (effective July 31, 2025), OTA Level II fieldwork is a minimum of 16 weeks full-time.

  4. 4

Apply to NBCOT and pass the COTA exam

    1-3 months after graduation

Apply online ($540), clear the character review, get your ATT letter, and schedule the 4-hour, 190-item exam at Pearson VUE within 90 days. You need a scaled score of 450 or higher.

  5. 5

    Get your state license

a few weeks, varies by state

Most states require OTAs to be licensed. Send your NBCOT score transfer to the state board and meet its extra requirements, such as a jurisprudence exam or background check. You practice under OT supervision as your state's practice act defines it.

  6. 6

    Renew and consider advancing

first renewal at 4 years, then every 3 years

Earn 36 professional development units for your first NBCOT renewal, due four years after certification, then renew every 3 years. Some COTAs later complete an OT program and sit for the OTR exam.

Step one: the right program

The program is the one step you cannot shortcut. The exam body checks where you trained before it lets you sit, so an unaccredited course can cost you the whole route.

Program: Associate degree (most common) or bachelor's degree from an accredited OTA program
Accreditation: Accreditation Council for Occupational Therapy Education (ACOTE), associated with AOTA
Typical length: Associate programs generally take 2 years of full-time study (BLS), including at least 16 weeks of full-time Level II fieldwork

The exam itself

This is the checkpoint the whole route builds towards. The essentials:

Exam: NBCOT Certified Occupational Therapy Assistant (COTA) Certification Examination
Questions: 190
Time: 240 minutes
Where you test: Computer-based, in person at Pearson VUE test centers (Pearson Professional Centers and PTC Selects); Pearson replaced Prometric on January 2, 2024. No remote/online-proctored option; the handbook describes the standard exam as taken in person at a test center.
Passing standard: Criterion-referenced; scores reported on a 300-600 scale and a total scaled score of at least 450 is required to pass. Passing standard set by a modified Angoff method.
Fee: $540

Licensure: what your state adds

NBCOT grants the national COTA credential, and state boards license OTAs. BLS says OTAs usually need a state-issued license that requires an accredited program, fieldwork and passing the exam. OTAs practice under the supervision of an occupational therapist as defined by each state's practice act. The OT Licensure Compact, which covers both OTs and OTAs, began issuing multistate privileges to practice in 2026 in a small group of member states.

Pay and job outlook

The US Bureau of Labor Statistics files occupational therapy assistants under “Occupational Therapy Assistants (31-2011)”. Median means half earn more and half less; pay moves a long way with state, setting and experience, so read this as a benchmark, not an offer.

Keeping the credential

Cycle: First renewal four years after the year you pass (renewal season January-March), then every three years
Requirement: 36 professional development units per cycle, abide by the NBCOT Code of Conduct, completed renewal application
Fee: $65 online / $75 paper renewal application; reinstatement $125 within 3 years of expiration, $175 after

FAQ

How long does it take to become an occupational therapy assistant?

Most people finish an accredited associate OTA program in about 2 years of full-time study. That includes at least 16 weeks of full-time Level II fieldwork. Then you pass the NBCOT COTA exam and apply for a state license.

How much do occupational therapy assistants make?

BLS reports a median annual wage of $72,300 for occupational therapy assistants in May 2025. It projects 20% growth for OT assistants and aides combined from 2025 to 2035.

What is the difference between a COTA and an OTR?

An OTR needs a master's or doctoral degree, evaluates clients and creates the treatment plan. A COTA needs an associate or bachelor's degree from an accredited OTA program and carries out interventions under the OTR's supervision. Each credential has its own NBCOT exam.

Can a COTA become an OTR later?

Yes. A COTA can enroll in an ACOTE-accredited entry-level OT master's or doctoral program. After graduating, they apply for the OTR exam through their existing MyNBCOT account.
