NBCOT certification is national and voluntary to keep, but all states license occupational therapists, and passing the NBCOT OTR exam is the standard exam requirement for initial licensure. The state board, not NBCOT, grants the right to practice. Some states issue temporary permits to graduates awaiting results, and those permits may end if the candidate fails. The OT Licensure Compact began issuing multistate privileges to practice in 2026, starting in a small group of member states.
